To RSVP to the ceremony, you must provide full legal name (first and last) and Driver's License, Passport, or Birth Certificate number (children without State I.D.) of ALL attendees in the "Notes" section. Carnival Cruise Lines will not allow you onto the ship, per Coast Guard and Homeland Security regulations, without this information.

Also, please indicate whether you are attending the "Ceremony," "Reception," or "Both."
